Three things. Photo ID in your own name (a resident registration card or driver's license; foreign residents use an alien registration card), something to verify your identity with (a credit or debit card in your own name), and a SIM card (USIM). If you do not have a SIM, some carriers will send one with your application. Make sure you are on a stable internet connection, such as Wi-Fi, when you apply.
